By signing this form I agree, acknowledge, represent and warrant the following:

1. I am 18 years of age or older.
2. I am a citizen or legal resident of the United States of America.
3. I am not a candidate for public office and I agree that I shall not become one for at least one (1) year after the initial broadcast of the Program(s) in which I appear, if I am selected to participate in the Program.
4. I have never been convicted of a felony.
5. Neither I, nor any member of my immediate family nor anyone living in my household is or has been an officer, director, employee, and/or agent of any of the following:
   1. Single Shot Productions, LLC (“SSP”), Rocket Science Laboratories, LLC (“RSL”), BermanBraun, French TV, American Broadcasting Companies, Inc. (“ABC”), or any television station owned or operated by ABC; Sullivan Compliance Company or Sky Castle Entertainment, Inc.; or any of the aforementioned entities’ respective parents, subsidiaries or affiliated companies.
   2. Any other entity or person involved in the development, production, distribution or exploitation of the Program.
   Additionally, SSP reserves the right, in its sole and absolute discretion, to disqualify any person SSP deems connected with the Program, whether in the Program’s production, casting, administration, judging, distribution, and/or exploitation, such that said person’s participation would create an appearance of impropriety. Any and all eligibility determination made by SSP shall be final and are not subject to challenge or appeal. Further SSP has no obligation to proceed with the production of the Program or any aspect thereof, including but not limited to the selection of any individual to participate in the Program. And neither ABC nor any television station owned or operated by ABC is obligated to broadcast the Program.
6. I affirm that I comply with all the eligibility requirements delineated in this questionnaire. I affirm that I am willing to submit to a background check and psychological testing.
7. I affirm and acknowledge that in the event that I am designated as eligible to participate in the program by SSP, that any such designation creates no obligation on behalf of SSP to include me in the Program and in the event that SSP asks me to participate in the Program that such participation in no way guarantees me, or obligates SSP to pay to me, any prize, incentive, or compensation of any kind in relation to the Program.
8. This questionnaire, the information contained herein and any other material, including but not limited to pictures, films and videotapes as well as the copyrights thereof, submitted by me or taken of me in connection with my application to participate (“Application”) on the Program become the property of SSP, and shall not be returned.
9. I hereby authorize and consent to the use, in any manner, in any and all media, whether now known or hereinafter devised, throughout the universe in perpetuity of my name, voice, likeness, any biographical information and any other material, including but not limited to pictures, films and videotapes submitted by me or taken of me in connection with my Application.
10. This questionnaire and Application in no way constitutes an application for employment, as no form of employment is being offered and I am knowingly and voluntarily submitting all of the information in this questionnaire.
11. I hereby release, discharge and hold harmless SSP, RSL, BermanBraun, French TV, ABC, and their respective officers, directors, employees, agents, affiliates, parents, subsidiaries, successors, assigns, affiliates, and/or licensees (collectively “Releasees”) from any claims, demands or causes of action (“Claims”) arising out of or in any way related to this questionnaire, any information contained herein, the Program and/or the use of any materials submitted by me or taken of me in connection with my Application, including but not limited to any and all such Claims for libel, defamation, intentional infliction of emotional distress, negligent infliction of emotional distress, alienation of affection, invasion of privacy or right of publicity, infringement of copyright, tortious interference, or violation of any other right arising out of or relating directly or indirectly to the my Application or any of the subject matter hereof.
12. I hereby acknowledge and affirm that I waive all rights to equitable and/or injunctive relief, including but not limited to, any equitable and/or injunctive relief against the broadcast, distribution, sale, purchase, and/or exploitation of the Program in any manner or media throughout the universe in perpetuity. I further acknowledge that this agreement shall in no way limit the provisional remedies available to SSP, including but not limited to injunctive relief.
